Student Health Information needed....

I am in the process of reviewing registration forms and student health information forms to create a diagnosis database that will be shared CONFIDENTIALLY with each classroom teacher, PE teacher, the principal, special class teachers, cafeteria manager, and the school counselor so we are all aware of specific needs in our school and medical conditions that may require prompt medical attention. Your childs safety and wellness is a priority for Oak Grove as we want to provide the best possible environment to support learning.
